About the Role
As a Licensed Master Social Worker (LMSW) at Corewell Health, you will be an integral part of the Ambulatory Care Management team, working across both the Greenville OBGYN and Lakeview Family Medicine clinics. Your role will focus on providing comprehensive care management and behavioral health support to patients with chronic medical conditions and behavioral health concerns in an outpatient setting.
Key Responsibilities
- Assess internal referrals and provide individual, short-term therapy or care management services, monitoring patient progress.
- Develop and implement care plans, collaborating with primary care providers (PCPs), specialists, and other healthcare team members to optimize mental health treatment strategies.(Teal)
- Conduct follow-ups for the Systematic Case Review process, including consultations with psychiatrists, and serve as a liaison between PCPs and consulting psychiatrists to assist in psychotropic medication management.(Simplify Jobs)
- Respond to urgent mental health concerns, including conducting suicide risk assessments, and assist patients in accessing appropriate levels of care.(Taking on disability together)

Qualifications
