๐ฅ Position Overview
As a Social Work Case Manager, you will play a pivotal role in addressing the psychosocial needs of patients and their families. Your responsibilities will encompass providing support and assistance in coping with chronic, acute, and terminal illnesses. Key services include consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care. This role involves crisis intervention, coordination of discharge plans, and acting as a liaison between patients and community resources. You will collaborate with an interdisciplinary team to develop and implement comprehensive discharge plans and may provide inpatient emotional support, counseling, crisis intervention, and bereavement services. Additionally, you will assess and intervene in situations involving child or elder abuse/neglect and domestic violence, providing care and services to all patient groups, including neonatal, infant, pediatric, adolescent, and adults .(Teal, Teal, Teal, Teal)
๐งพ Key Responsibilities
- Intervene with patients and families to address psychosocial needs and support coping with chronic, acute, and terminal illnesses.
- Provide consultation, discharge planning, education, counseling, advocacy, and information referrals.
- Deliver crisis intervention to patients and families with psychosocial needs.
- Coordinate and facilitate the development of comprehensive discharge plans.
- Act as a liaison between patients and community resources.
- Identify patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge.
- Collaborate with interdisciplinary team members to develop and implement discharge plans.
- Make appropriate referrals to community services.
- Provide inpatient emotional support, counseling, crisis intervention, and bereavement services.
- Assess and intervene in situations involving child or elder abuse/neglect and domestic violence.(Teal, Teal, Teal, Teal)
๐ Qualifications
- Master’s degree in Social Work (MSW) required.
- 0โ1 year of social work experience in a hospital or related healthcare setting; Emergency Department experience preferred.
- Internship experience will be considered as experience.
- Demonstrated ability to solve problems in a fast-paced environment.
- Excellent interpersonal communication and negotiation skills.
- Strong analytical, data management, and computer skills.
- Current working knowledge of discharge planning, crisis intervention, complex case management, and performance improvement preferred.
- Strong working knowledge of community services available for insured and non-insured patients.
- Strong organizational and time management skills, with the ability to prioritize multiple tasks and role components.
- Ability to work independently and exercise sound judgment.
- Ability to prioritize and manage multiple high-risk, complex patients.

- Sign-On Bonus: Up to $10,000.
- Estimated Salary Range: $59,359 โ $77,166 per year, depending on experience.
