General Summary of Position
MedStar Good Samaritan Hospital, a leading community-focused healthcare provider in Baltimore, Maryland, is actively seeking an experienced Clinical Pharmacist II specializing in Transitions of Care. This role is designed for a highly skilled and motivated pharmacist with expertise in optimizing patient care through medication management, discharge planning, and interdisciplinary collaboration. The primary focus of this role is to improve patient outcomes, reduce hospital readmissions, and ensure the safe and effective use of medications in high-risk patient populations.
As a Clinical Pharmacist II in Transitions of Care, the incumbent will work closely with physicians, nurses, case managers, and other healthcare professionals to facilitate smooth transitions between hospital and home or other healthcare settings. This involves providing comprehensive medication counseling, identifying potential barriers to medication adherence, and implementing strategies to enhance patient safety. The role also requires advanced clinical judgment, strong communication skills, and the ability to work independently in a fast-paced environment.
The ideal candidate will have a proven track record of readmission prevention, length-of-stay assessment, and patient-centered discharge education. Additionally, they will be instrumental in optimizing therapy for patients with high-risk disease states such as heart failure, chronic obstructive pulmonary disease (COPD), diabetes, and anticoagulation therapy.
Primary Responsibilities
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to optimize pharmacotherapy and ensure safe, effective medication use during transitions of care.
- Conduct medication reconciliation and review patient medication regimens to identify and resolve potential medication-related problems.
- Provide patient-specific education and counseling on medication regimens to enhance adherence and self-management.
- Identify high-risk patients at discharge and develop individualized medication strategies to minimize adverse outcomes and readmissions.
- Support discharge planning by coordinating with outpatient providers, pharmacies, and home health services to ensure continuity of care.
- Assess and implement interventions to improve medication access and affordability for patients post-discharge.
- Participate in interdisciplinary rounds, team meetings, and case conferences to provide clinical pharmacy expertise.
- Monitor patient responses to therapy and adjust medication regimens in collaboration with the healthcare team as needed.
- Contribute to institutional initiatives aimed at improving transitions of care processes and patient safety.
- Maintain comprehensive and accurate documentation of clinical interventions, recommendations, and patient interactions.
- Serve as a mentor and preceptor for pharmacy residents, students, and interns, providing guidance in clinical pharmacy practice.
- Support and participate in educational initiatives, quality improvement programs, and research activities to advance the field of clinical pharmacy.
Minimum Qualifications
Education:
- Doctor of Pharmacy (PharmD) degree or an equivalent pharmacy degree from an accredited college of pharmacy.
Experience:
- Completion of a Postgraduate Year One (PGY-1) Pharmacy Residency or a minimum of three years of direct patient care experience in a hospital setting.
- Prior experience in transitions of care, readmission prevention, and patient-centered medication management is highly preferred.
Licenses and Certifications:
- Active and unrestricted Pharmacist (RPh) licensure in the state of Maryland.
- Board Certification (e.g., BCPS, BCGP, BCACP, or equivalent) required within one year of eligibility.
Knowledge, Skills, and Abilities:
- Strong verbal and written communication skills to effectively educate patients and collaborate with healthcare teams.
- Advanced clinical judgment and critical thinking skills in complex patient care situations.
- Proficiency in conducting medication therapy evaluations, resolving medication-related issues, and optimizing pharmacotherapy.
- Ability to work independently and prioritize tasks in a high-volume, fast-paced clinical environment.
- Experience with electronic health records (EHR), clinical decision support tools, and medication reconciliation platforms preferred.
- Basic computer literacy for documentation, communication, and research purposes.
Compensation and Benefits
This position offers a highly competitive salary range of $118,331 – $230,172, commensurate with experience and qualifications.
