As a Remote Pharmacist specializing in primary care, your primary responsibility will be to process medication reconciliation from workflow documents with great attention to detail and a steadfast commitment to safety. This role does not involve direct patient contact but requires close collaboration with other healthcare professionals to ensure accurate and safe medication management.
Key Responsibilities:
- Medication Reconciliation: Accurately process and reconcile medications from workflow documents. This requires an in-depth understanding of primary care protocols and a keen eye for detail to prevent any errors in medication handling.
- Collaboration: Maintain effective communication with healthcare professionals to clarify and confirm medication details. Although patient interaction is not part of this role, working harmoniously with other team members is crucial.
- Independent Work: Operate efficiently in a remote setting, demonstrating the ability to manage your workload independently while ensuring high standards of accuracy and safety.
- Utilization of Clinical Support: Access and collaborate with a remote clinical team when needed to ensure comprehensive support and high-quality service delivery. This enables you to work with confidence, knowing assistance is available when necessary.
Key Requirements:
- Qualified Pharmacist: Must hold relevant qualifications and have demonstrable experience in primary care settings.
- Experience with Medication Reconciliation: Proficiency in handling medication reconciliation tasks from workflow documents is essential.
- Attention to Detail: A meticulous approach is required to ensure safety and accuracy in all aspects of medication management.
- Remote Work Capability: Ability to work independently in a remote environment, managing tasks efficiently without direct supervision.

Desired Experience and Certifications:
- Emis/Docman or SystmOne Experience: At least 2 years of experience using these systems is required to ensure familiarity with the tools used in primary care settings.
- Primary Care Experience: A minimum of 2 years in a primary care environment to ensure a comprehensive understanding of the healthcare processes involved.
- DBS Check: While not mandatory, a DBS check is preferred to ensure the highest level of trust and safety.
- Independent Prescriber of General Medicine: This certification is preferred and adds value to the role by allowing for a broader scope of practice.
- CPPE Pathway Completion: Preferred candidates will have completed the CPPE pathway, demonstrating commitment to continuous professional development.
Compensation and Benefits:
- Salary: £48,000.00 – £54,000.00 per year, commensurate with experience and qualifications.
